    Abdominal Aorta Dissecting Aneurysm, Acute Pancreatitis &.Myocardial Infarction

    Emergency-Room Case Report: a male patient, 61y, complained of sudden severe pain in the left side of his back which diverged to the back of sterna and epigastrium after supper, accompanied by perspiration and fever.(Acute Pancreatitis? Myocardial Infarction?)On general examination we found his blood pressure was 170/100 mmHg, his heart rate 84 times/minute, his heart rhythm was regular and his pulmonary field was clear. However, there were tenderness and rebound tenderness in the left side of his abdomen. After the treatment with antispasmodic, analgestic and hypotensor and 3-day's emergency observation, the abdominalgia had relieved a little, yet the tenderness and rebound tenderness still remained. After admission, vascular murmur was found in the left side of the abdomen and there was also increase of the echo irregularity of the abdominal aorta paries on echocardiogram. Diagnosis: Abdominal Aorta Dissecting Aneurysm!

    A person you should know — Dr. Buckminster Fuller

    R. Buckminster Fuller, known by his friends as "Bucky", has undeniably been one of the key innovators in the 20th century. He is known as a philosopher, thinker, visionary, inventor, architect, engineer, mathematician, poet, cosmologist, and more.

    Buckminster Fuller was probably one of the first futurists and global thinkers. He is the one who coined the term "Spaceship Earth", and his work has inspired and paved the way for many who came after him.

    This is the Dymaxion Map of the world, developed by Fuller as the first world projection to show the continents on a flat surface without visible distortion. Furthermore, this view shows the earth as being essentially one island in one ocean.

    Bucky was the person most responsible for making Synergy a common term. Much of his work was about exploring and creating synergy. He found synergy to be a basic principle of all interactive systems. He developed a subject called Synergetics, a "Geometry of Thinking".

    Fuller is the inventor of the Geodesic dome, and was a pioneer in utilizing basic geometical shapes in design.

    A key goal for Buckminster Fuller was the development of what he called "Comprehensive Anticipatory Design Science", which is the attempt to anticipate and solve humanity's major problems by providing "more and more life support for everybody, with less and less resources."Fuller routinely demonstrated his ideas in what he called "artifacts", tangible prototypes or models of designs and principles.

    Acute Abdomen and Myocardial Infarction

    Emergency-Room Case Report: a male patient, 58y, complained of abdomen pain for 1h with nausea but with no vomit. Past history: gastritis and gastroenteritic ulcer for 20 years. “Acute Abdomen?” PE: Vital T37, P100, R22, BP 100/60mmHg, the patient was a well developed, well nourished adult male apparently in acute illness, with pallor, asthenia palms, a cool, moist skin. No distended neck veins, no cyanosis. Examination of the chest revealed a few scattered pulmonary rales. The left border of cardiac dullness extended 1 cm to the mid-clavicular line in the fifth intercostal space. The apical impulse was diffuse, and the rhythm is irregular. The cardiac sounds were of faint quality, the aortic and pulmonic second sounds were equal. A grade 1 systolic murmur was audible at the apex. There was no gallop. Abdominal wall was somewhat thick with fatty deposit. No muscular defence or tenderness. Liver and spleen were not palpable. The remainder of the physical examination was unremarkable. “Myocardial infarction?” I said to myself. Yes! Electrocardiogram showed: 1) Ventricular premature beats. 2) Left ventricular  hypertrophy. 3) Pathological Q wave in V2~V4 , ST-T change in V1~V4. Diagnosis of  acute anterior wall myocardial infarction confirmed! Isosorbide mononitrate, analgetics and antithrombotic-thrombolytics had to be prescribed. soon the patient’s situation was under control, and I felt very glad that he got the right treatment before it was too late.

    My No.1 Priority --- Gainning working experiences as much as possible

    Intensive-Care-Unit Case Report: A patient M 58y, appeared Low Cardiac Output Syndrome after Aortic Valve Replacement Operation. The pericardiac and mediastinal blood drained from the patient’s chest tube showed no sign of bleeding slowing , meanwhile Arterial Blood Gas Analysis indicated PaO2. After blood transfusion, hemostasis treatment, and lung ventilator regulation, the patient’s pericardiac and mediastinal blooding was under control, and PaO2 was improved. Nevertheless, his BP declined to 73/48mmHg! This phenomenon may due to (1) Cardiac Failure or (2) Hypovolemic Shock. Although the patient’s urine volume is 22ml/h, his CVP is only 4cm. So I continued mass transfusion, and made it faster in order to volume expansion instead of using cardiac stimulants. About half an hour later, his BP had been elevated to 90/60mmHg.
